ORDERING INSTRUCTIONS


This form is for checks and money orders only. On-line orders should be made by clicking on the "PayPal" button on the previous item description page.

Please read and follow these easy steps:
1) Download the order form, then print it.
2) Fill out the order form and total your amounts.
3) Pennsylvania residents must add 6% for state sales tax.
4) Total it all up, take the order form and total payment and mail it to:

- Please allow 2 to 4 weeks for your order to be received and processed.
